Posterior Capsular Opacification (PCO)

What is Posterior Capsular Opacification (PCO)?

Posterior Capsular Opacification (PCO) is a medical term used for ‘secondary cataract’ that in reality is not the real cataract but affects those undergoing cataract surgery under which natural lens of their eye is replaced with an artificial intraocular lens (IOL) following which the thin membrane (posterior capsule) that holds the new lens in place may become cloudy over time causing the eyesight blurred or hazy. According to medical experts, around 20-50% of patients undergoing cataract surgery are affected with PCO or secondary cataract generally several months/years after their operation.

Posterior Capsular Opacification (PCO)

Symptoms:

Following are the most common symptom of Posterior Capsular Opacification, a gradual decline in vision/eyesight, matching the symptoms of cataracts.

  • Blurred or hazy eyesight.
  • Glimmer or coronae around light/rays, particularly in darkness.
  • Hardship reading or performing activities that need precise eyesight.
  • Sensitivity to shining rays or light
  • Less disparity of sensitivity

Causes:

PCO condition erupts when the remaining epithelial cells after the cataract surgery relocate, increasing on the posterior capsule, forming a cloudy layer, which blocks or disperses light passing through the eye, diminishing the clearness of vision or eyesight, making those susceptible to its effects who have had undergone cataract surgery, but, such condition is common in younger patients and those with certain types of intraocular lenses.

Diagnosis & Tests

Diagnosis of Posterior Capsular Opacification is straightforward and typically made during a routine eye exam by an ophthalmologist or optometrist:-

  • The slit-lamp examination focuses on a narrow beam of light onto the eye, which helps detect the presence of PCO by observing the clouding of the posterior capsule
  • Visual acuity tests help assess the decline in visual clarity, further confirming the diagnosis

Management & Treatment

The most effective treatment for Posterior Capsular Opacification is a procedure known as YAG laser Capsulotomy, which is a quick and manageable outpatient procedure in which the cloudy hindmost capsule is treated with a laser, creating a small space, allowing light to pass through to the retina without obstruction.

YAG laser capsulotomy:-

  • Takes only a few minutes.
  • Is non-invasive and generally does not require anesthesia
  • Patients can resume normal activities shortly after the procedure
  • Vision improves almost immediately or within a day after treatment

In rare cases, complications like retinal detachment or increased eye pressure can occur after the YAG procedure, but these are uncommon

Prevention

Presently, there is no medically approved method to control Posterior Capsular Opacification, as it is considered a naturally occurring effect of post-cataract surgery, but modern surgical techniques and advancements brought in intraocular lenses, including the use of lenses with sharp edges, have reduced the incidence of PCO. Hence, regular follow-up appointments after cataract surgery are crucial for early detection and timely management of PCO.

Outlook / Prognosis

The prognosis for patients with PCO is excellent as the patients, once treated with YAG laser capsulotomy, the best procedure considered safe and effective, yielding long-lasting results, most people feel a rapid and significant improvement in their eyesight or vision, and a negligible percentage of patients require further medical intervention.

Living With

After treatment for Posterior Capsular Opacification, most patients can return to their regular activities, including reading, driving, and other tasks requiring clear vision. It is important to continue attending regular eye exams, especially for those with other eye conditions such as glaucoma or macular degeneration. Additionally, patients should monitor for any changes in vision and report them to their eye care provider promptly.
